To receive your driver record, press the Download Driver Record Request Form button. Complete the required information on the application and fax to (713) 488-4102. Upon receipt of the completed application, we will process your request and you will receive your driver record within 10-15 business days. If you cannot fax the �Application for Copy of Driver Record,� you may mail the application to:
